• UK residence permit (original and copy) must be valid for at least 90 days beyond the return date of the trip to the Schengen Area.
• Two recent passport size photographs (not older than 1 month) taken against a white background.
• Visa application form, dully completed in all sections, dated and signed. Application form is in the download pack.
• EEA/EU and Swiss national’s passport (original and copy).
• Marriage/ birth certificate (original and copy). If not in English or Italian then translation by a certified translator in the UK of the same is required.
• Fully paid return flight tickets mentioning both applicant and EEA/EU and Swiss national family member’s names.
• Completed GAN Visa Order Form which is in the download visa package.
